Job Summary

Responsible for the selection of applicable Diagnoses and Procedures, sequence codes following the ICD-10-CM/PCS official coding guidelines. Query the provider (physician or other qualified healthcare practitioner) for clarification and/or additional documentation when there is conflicting, incomplete, or ambiguous information in the health record regarding a significant reportable condition or procedure or other reportable data element dependent on health record documentation (e.g. present on admission indicators). Advance coding knowledge and practice through continuing education, including but not limited to meeting continuing education requirements.

Duties & Responsibilities

• Assign codes to diagnoses and procedures using ICD (International Classification of Diseases) and CPT (Current Procedural Terminology) Codes primarily for Inpatient services but also assist with observation services, outpatient surgical services, and other services as needed.

• Responsible for accurate assignment of Present on Admission indicators

• Assist with accurate capture and reporting of data fields such as discharge disposition.

• Ensures codes are accurate and sequenced correctly in accordance with government and insurance regulations. • Communicate (Query) with the medical staff for any documentation that is insufficient and/or unclear.

• Able to work independently

• Understands and works coding edits and bill holds.

• Flexible in assuming other appropriate responsibilities not noted above.

Skills & Qualifications

• Experience – One or more years of hospital inpatient coding experience preferred.

• Licensure – Certification with AHIMA (RHIA, RHIT or CCS) required.

Education • Two-year degree in Health Information Management or related field preferred.

Physical Requirements • The position requires a high degree of mental and visual concentration. No lifting requirements. Requires long period of sitting, with repetitive motion.
